Your Appointment
For all appointments, please remember:
- Arrive 15 minutes early.
- Bring all prescriptions and over-the-counter medications you are taking.
- Bring your insurance card and co-payment.
- Tell the check-in staff if you don't have insurance or need help paying for services. You may qualify for discounted fees based on income/family size. See Sliding Fee Program for more information or ask the staff at any of our locations.
- Call 24 hours in advance to cancel or re-schedule an appointment to avoid a $25 fee. Failure to cancel 24 hours in advance is considered a "no-show" appointment. Multiple "no-show" appointments may result in dismissal from the practice. See No Show Policy for more information.
