HHS provides a Sliding Fee Discount Program to reduce barriers to accessing care by offering services at a discounted fee to those that qualify. This program is available to those with and without insurance. Eligibility is based on family size and income.

To apply for the sliding fee discount program patients must:

1. Download and complete the Sliding Fee Discount Application. Paper applications are also available at all HHS locations. An application can be mailed to you, upon request. 

2. Provide the required proof of income for all household members.

3. Return the completed application, along with all required documentation, to any of our centers for processing. Applications can also be mailed. Mailing addresses can be found on page 1 of the application.

Patients cannot receive the sliding fee discount for services until enrollment in the program is completed.
